Steps to Claiming your Free Hats
- We’ll confirm your subsidy balance and details with the Ministry of Health.
- Send Us These Two Documents:
- MOH Hair Loss Subsidy Claim Form: Download, fill in, and sign it Here.
- Medical Certificate: Signed by your doctor or nurse (this may have a sticker).
Scan or photograph and email these documents to kerrin@kaushats.co.nz. Make sure they’re clear and include all details.
Important:
- When filling out the form, include the shipping cost in the subsidy amount. If not, you’ll need to resend it.
- For subsequent orders, just refill the same form, tick "subsequent order," and resend it. No need for another medical certificate.
Note:
Second orders may take longer as we need to wait for MOH processing.

We’ve made it easy with a no-payment option:
- Select the Bank Deposit & MOH Payment option at checkout. This lets you save your hats while we wait for your documents—no payment needed upfront.
Once everything’s sorted, we’ll send your hats—simple as that!
